About a week before Thanksgiving the truck was running great and I took it to The Old Dominion C10 Rally. I replaced the battery prior to the rally as it was slow to turn over one morning when the temp had dropped.
After parking the truck post rally, it rained a couple days and I didn't drive it for about 3 days. Driving it for the first time in 3 days I was not surprised it was running rough. Since then the weather has rendered continual driving a few more times. It is currently running rough when I drive it.
For a while I was convinced the truck needed to drive a few days in a row and only have a day of no driving break.
It definitely has shown it drives better when driven more than not.
The last time this happened I panicked and called my wife to follow me in the truck from the Dr's Office parking lot I was in that day to the garage. This panic came on when taking my son to the doc for an appt. The truck barely made it and cut off on us at a couple stop signs, but we managed to limp there. When the better half showed up I cranked it and it fired up good and strong. Driving from the Dr's office to the garage, the truck drove great. When arriving at the garage a convo happened and we agreed since it was running well currently, I would keep an eye on it and come back when needed.
So now back to current days, it is running rough. I will warm it up real good(9-10 minutes) for a drive and it can run well around the neighborhood but at some point when the truck gets to going between 25 and 30 mph it stutters, and clunks, and seems as if it is not getting fuel, then will drive normal for a few moments. The same thing will happen when it approaches 35 to 45 mph.
Recently it barely turned over after driving it to the grocery store. This is post battery change.
So these are symptoms that have come up in the past but were thought to be taken care of.
I wish I could get some of you guys to just ride in the cab with me for a few days to see and hear what is happening. I believe someone is out there that could figure out exactly what this TBI-SBC needs for help. All kinds of first thoughts running around in my head for where to begin, but I feel like I am starting to really need to ask somone that knows more than me to start troubleshooting.
Maybe:
-Fuel Pump
-Fuel filter
-Pump at the block
-TPS
-IACV
-Alternator(slow turn over after new battery)
At a loss. I just want to be able to have a good ride when needed. Also, funds are tight and the fun truck is not family priority currently.
When the weather permits(currently 5 inches of snow and counting) I will be firing it up to test run some more.

Check your battery voltage then start the truck and check the voltage again.post results. Turn off everything in the truck remove the negative battery cable put a test light in between the battery and the negative cable does the test light light? If so disconnect the alternator, does the light go out?

Man you can’t catch a break w that truck.
Parasitic load on the battery (what Rick told you to check) is not related to running rough. Assuming your alternator is charging. Even then it should run fine until the battery runs down. It would be repeatable.

Not true a bad alternator or alternator with bad diodes can cause rough running. A bad diode can induce AC ripple which will confuse the ecm. Also if voltage isn't staying high enough fuel pressure can be wrong so can injector duty cycle. I sure hope he an 87 like I was thinking. LOL

Bad plug? Bad plug wire? Maybe just loose?

Fair enough. True since he has a TBI ECM. Had that happen on a much newer vehicle once
@JamesSam your alternator could be both issues. Bad diode(s) or rectifier can/will create a current draw. Common in reman alternators or old ones.
And affecting the electronics ECM etc can be random.
Easy test, check for current draw. If alt, replace.
And or fully charged battery, start truck. If rough running unplug alt and see if it corrects (instantly).

Reman alts are often junk. Sometimes immediately. Reman doesn’t replace working parts like rectifiers if they test good. Then you got a 30 year old 150k mile rectifier in a shiny reman.

That's why I avoid Auto Zone.
