My 89 R3500 driver side mud flap mount is torn and it makes a ton of noise when driving. I've been looking for a direct replacement but I've not had any luck finding anything that looks like it will bolt right up, does anyone have any suggestion?

Those are fabricated. You'll have to make a new one or weld that one back together.

It would be less work to weld that one rather than building from scratch. If it was mine, I'd weld an additional piece of flat stock to the top or bottom of the repair to prevent a reoccurrence.

I built a set for mine back in the early 90's as no one made a set for 24" wide mud flaps. Most kit at the time were only 18" wide.

I used a "L" shaped piece of stock and cut a wedge out of the corner, then welded it at 90 degrees. I used 2 of the bed bolts holding in the the well wheel at the bed floor to attach it to the truck, then welded a piece of flat stock to the perpendicular section holding the flap and attached it to the bed bolt marked by the red arrow.
