Well, I have been working on the ole' TBI 350 the past 3-4 weeks, trying to get it running smoothly. Some things have helped, like new vacuum lines, other things havent made a difference, but still were a good idea (a few new sensors, rebuilt the TB, etc).

Today decided that maybe a clogged cat was the problem, so cut the old one out and for fun, fired it up. My exhaust has been very quiet before, but it sounds really cool with nothing on it. Right away heard something wrong - missing on a cylinder at lower RPM. Spark plug wires and coil are fine, fyi. We think its either a burned valve, worn lobe on the cam, or something with a lifter.

Anyways, for now, just going to drive it. Maybe look into some Vortec Heads and a cam to replace these in a few months.

Are you lacking power or just running a bit rough?

Exhaust modifications will screw up the EGR systems. Unplug the EGR valve and see if it smooths out.

There's a sticky at the top of this section with a really great site on understanding what a vacuum gauge is saying. It might give you a look at the problem from a different angle.
