What are syptoms of a failed/clogged Cat?

Smell.
Idles good & smooth but loss of power throughout powerband without any real "miss".
Carboned up plugs.

How about engine temp heating up at higher rpms under heavy acceleration? That could be a lean issue too right?

Excessive Exhaust temperatures
Rotten egg smell
Transmission doesn't want to shift out of second gear
Poor Exhaust flow out of the tailpipe.
When under a load converter can be red hot.

In theory the engine temperature could run warmer then normal, because the hot exhaust doesn't escape the tailpipe and back flows to the engine. But the engine cooling system may very well disguise this.

All of those symptoms still aren't a sure sign, just a good indicator. I once watched a guy replace a perfectly good cat (still looked new) because the car lacked power and the cat would glow red. I later found the TPS to be incorrectly installed. Well, I found many other things wrong, but the TPS was the big culprit.
