The sensor does thread into the collector a few inches pre-flange. Is it still too far off?

Even if you had mid length headers, its too far.
1 wire for shortys or stock manifolds, 3 wire heated for anything else.

Well, maybe.
I want to make sure you have everything set up properly for that particular exhaust setup. The 3 wire o2 MIGHT be all you need to get passed smog.
The one wire sensor is probably sending your ECM into open loop, which causes it to revert to factory settings for a cold engine, which is most likely too rich for an engine thats at operating temp.
But, like I said before, you may want to have a little talk with your smog guy, and see if he would be willing to help you with some sniffer tests to get the truck right. If he's a nice guy, and sees that you are trying to correct the problem, he may be willing to help.
Also, a 195* thermostat is very important for the same reason, the cooler the engine runs, from what is normal factory temps, the ECM will add a little more fuel than necessary.
And dont forget the laquer thinner, and a good drive in 3rd. this is to help clean up your cats, which may be at low efficiency, due to the rich conditions that you have been driving under.
All of this should mak a drastic improvement on the smog machine, but if its not enough, then we can search for another chip, which wont be difficult at all. Oh yeah, after you get the new o2 wired up, and confirm the thermostat, disconnect your battery for 15 seconds(you should really disconnect it before working on the truck) so the ecm relearns the engine with the new sensor.

no, the black wire is the sensor to ECM wire. the white wires ar the pos and neg for the heater element inside the sensor.

Jim, are the computers in similar model years full size vans the same as the trucks? I think some of those vans I saw today might still have some computers. I didn't really focus on it but it is something I can look for next time on the TBI vans. I got to start paying attention to that and even start grabbing some sensors off those things.

Hmm. I still have the stock chip in my '88 4x4 Burb, I removed the smog pump though. Only problem is I need a chip in the truck to keep it going everyday. If 89S's extra would work in mine, I could stick it in there and then send my stock chip to Brandon.

Would that work?

Sure would!
I dont know if his SC is a stage 1 or 2 though, but a stock gm Calibration might make the sniffer happy, along with a 3 wire o2.
I think his 1 wire O2 sensor being 50 miles downstream from the engine is his big issue though.
