Those look like the aftermarket replacement heads. My 1987 V3500 came with them from GM. I have owned the truck since 1988. The original GM heads have the arm going inside the backing.

I am currently restoring my truck and removed the original mirrors and am installing the "below eyeline" mirrors. Commonly called Dumbo mirrors.

Those are aftermarket mirrors you are looking at, they are not original GM year specific mirrors. The arms and mounting locations are not even the same. They don’t make them anymore. You can only buy used originals. I am 60 years old and I have owned nearly 20 of these trucks, most all of them K30.

1974 brochure linked above, you are completely wrong. They were optional and installed at the factory. Look at "camper mirrors".
