anybody out there try to put nv4500 into an truck with automatic. has 203 transfer case but don't see any adapters for this conversion

Welcome to the site. You could get a 205 as well.

I think the only t-case options for behind a 4500 is a 208, 241 or a later (round pattern) 205.

Yep, that's why I listed the three that I did. If you want a simple shifter poking through the floor like the 203, you will need to run a 205. Or divise a custom shifter for the 208/241.
