Years ago I added Air Lift Air Bags to the front of my C20 as I wanted to run 33x12.50 tires. It gave me about 2" of lift. This is the kit for your truck:

Another option is look at the lift spindles for a "B" body GM car. They may work. Caprice (police version 9C1) and full size RWD Cadillacs had 12" rotors with 5 on 5" lug patterns.
