Smaller, less populated areas and counties. I know of couple of 'em, lived in one of 'em before I moved here. If the county has any city with a few hundred thousand, then whole county has to test from what I understand

There are quite a few areas where testing is only required at transfer of title. If a family member is transferring it to you, I believe the test at transfer of title is waived.

You can't go wrong with going back to original. Enjoy it for what it is. Maybe an overdrive transmission. It's a real pain to go after market and you're going to spend a lot more money/time. Just my 2 cents. Post some pictures of your truck. Cheers from Goodyear, AZ
