My best guess would be wrong torque when it was put in at some point. Too much torque cracked the prong and the bolt backed out with nothing to hold on.

Hey man, if your small block has a mechanical pump you don't need a electric one. The mechanical ones will last forever. Why did you block off and switch?
Just my opinion but the rubberized coating is extremely hard to get back off. I used these 2k products from eastwood. They are ceramic based 2 part so its tough as nails.
