okay guys. ive been reading up on this nv4500 swap. found out that its best to find it with a tcase. my question is i have a nv3500?(in a 1/2ton 90's style truck) it has the transfer case and everything. would this go together the same way as the nv4500 swap? thanks for any input

If the 3500 came with a 241 behind it, that should work fine. You would use the 241 adapter. You would probably need some amount of suspension lift though, to keep the front shaft from hitting the slave cylinder.

If he has an 87 the left had drop wont line up with the axle. From what i get hes wanting to use a newer tranny in an older truck and hes going to have to change the t-case for the correct side output. So he will have to adapt the 208 to the newer NV3500

Yep, the RH drop 208 should bolt right up to the LH drop 241 adapter. It's been done with the 4L80E swaps, I don't see why it wouldn't apply here. The issue here is that with the front shaft now on the RH of the tranny, it can interfere with the slave cylinder since it is also on the RH side. A 4 inch lift should clear it fine.
