    My best guess would be wrong torque when it was put in at some point. Too much torque cracked the prong and the bolt backed out with nothing to hold on.

    Before I'd hack the dash out I would buy a 2 knob Bluetooth unit. It weights nothing and just mounts using the knob holes. Fm/Bluetooth to the phone.

    They sell uv die you can add to oil. Get a black light and find the true source of the leak. There are multiple sources that could cause a bad leak.
