Again...yes, you do need a ground and a power source for the cigarette lighter aka power terminal to work...its how dc power works.
smoothandlow, if you're disagreeing with me on the need for a ground wire, the ash tray housing is bonded to the dash via the pivot pins and springs. The dash is bonded to the body.. And lastly, the body is connected to the battery negative via ground wires and the negative battery cable.. So no - no ground wire is needed.. If your post was made to emphasize my post, then disregard this post...

The lighter pulls more amps than a cell phone charger . Plugging in a carcharger under normal circumstances won't blow the fuse unless themcharger itself is shorted out internally (I had this happen in my wifes car...popped fuse...ended up the charger was junk).

Now, if you use the cigarette lighter power outlet to charge say a inverter, then yes, it will get overloaded

Yes...I was emphasizing your post regarding a ground on a 12v dc circuit.

I agree, long term use for a simple phone charger : no problemo.

But using it long term for something like a 400w power inverter: es no bueno.

What happens when you plug your charger in to it? Could be a faulty lighter button.
